This Service Agreement is made between S & H Tax Accountants and the Participant or their Authorised Representative under the National Disability Insurance Scheme (NDIS).
This Agreement will be effective from the date of acceptance and will remain in force for as long as the Participant continues to engage with S & H Tax Accountants, unless otherwise terminated in writing by the Participant or Authorised Representative.
To begin receiving services from S & H Tax Accountants, the Participant or Authorised Representative must provide their NDIS plan details, NDIS number, and any other required information as outlined in this Agreement.
Fees Schedule
S & H Tax Accountants will provide Plan Management and Financial Administration services to the Participant, as detailed in the attached Fees Schedule. All prices are inclusive of GST (where applicable) and cover the cost of the specified services. Any additional costs (i.e., expenses not included in the Participant’s NDIS supports) will be the responsibility of the Participant or Nominated Representative and are not part of the service fees.
S & H Tax Accountants agrees to:
S & H Tax Accountants will claim a monthly fee directly from the NDIA for the provision of support as outlined in the Fees Schedule under Improved Life Choices (Support Area 14), upon acceptance of this Service Agreement.
By nominating S & H Tax Accountants to provide plan management services and handle the funding, we will claim funding from the NDIA portal up to the amounts specified in the Participant’s current NDIS plan, within the approved Support Area and budget. After the supports have been provided, the service provider or Participant/Authorised Representative will submit a claim for payment to S & H Tax Accountants for those supports.
The Participant or their Authorised Representative will provide S & H Tax Accountants with details of the support budgets as outlined in the Participant’s current NDIS plan. If there are any changes to the support categories or budgets, the Participant or Authorised Representative must notify S & H Tax Accountants in writing, with the update signed and dated by the Participant or their Authorised Representative.
The Participant or their Authorised Representative agrees to promptly notify S & H Tax Accountants in writing if the Participant’s NDIS Plan is replaced with a new plan or if the Participant ceases to be a participant in the NDIS, and to provide any relevant plan details.
Either party may terminate this Service Agreement by providing one month’s written notice. In the event of a serious breach of this Agreement by either party, the notice requirement will be waived.

Nothing in this Service Agreement limits or negates the statutory guarantees regarding the provision of services as outlined under the Australian Consumer Law (Competition and Consumer Act 2010 – Schedule 2).
S & H Tax Accountants assumes, in good faith, that all information provided by the Participant or their Authorised Representative is accurate and true. Claims presented to S & H Tax Accountants are assumed to accurately reflect the goods and services provided to the Participant in accordance with the NDIS guidelines (National Disability Insurance Scheme Act 2013).
